Salford Together is an alliance between Salford City Council, Salford Royal NHS Foundation Trust, Salford Clinical Commissioning Group, and Greater Manchester West Mental Health NHS Foundation Trust. The alliance has developed an Integrated Care Programme for Older People in Salford.
Part of the programme involves mapping and developing communities and creating greater local resilience. This is led by the community assets group which seeks to strengthen local support networks already in place and also to help communities work together to find solutions to those problems that currently prevent older people remaining active and independent members of their community. The group also aim to address the barriers which affect older people and increase the risk of social isolation, loneliness or depression.
The barriers identified are:
- Limited physical activity
- Lack of access to information
- Not eating well
- Not engaged in activity
To reduce these barriers, the community assets group have been working on several projects:
- Postural Stability and Step-Up classes
- Tech and Tea
- Eating Well in Later Life
- Volunteer Wellbeing Champions
Salford CVS is working in partnership with Inspiring Communities Together to help deliver aspects of the Salford Together community asset programme.
Partnership projects include:
- Volunteering in Care Homes
- Developing a network of Volunteer Wellbeing Champions
- Tech and Tea
Visit the Salford Together website for more information on the community assets group.
