The Role

As a senior member of the locality team, you will be responsible for the effective delivery, monitoring and evaluation of our  social prescribing service, a key part of our Community Wellbeing Programme. You will work closely with the Strategic Locality Lead, our other managers, our public sector colleagues, and   commissioners, our local VCFSE, our staff and our clients to ensure we deliver a high-quality person-centred service, are responsive and adaptive and are delivering against our commitments and values.

The main things you will be asked to do in this role:

Developing the service and our staff

  • Managing and supporting a team of Social Prescribing Officers, PCN Link Workers and administration staff, providing them with all the support to enable them to provide the highest possible standard of delivery.
  • Continuing to develop the offer so that it best meets the needs and aspirations of our clients and our stakeholders and compliments the wider wellbeing offer in Tameside.
  • Establish and implement systems and processes to ensure we can deliver a high-quality offer to our members, monitor our impact and meet contractual obligations.
  • Ensure that service developments and outcomes are appropriately monitored, evaluated, and communicated.
  • Providing quality reports, presentations, and verbal updates to a variety of stakeholders internally and externally as required.
  • To ensure that the programme meets all its duties relating to the protection of data, including ensuring we remain Cyber-Essentials compliant, and our practices reflect the General Data Protection Regulations.
  • To be the locality Safeguarding Lead for Action Together in Tameside, taking appropriate action when these issues arise and communicating this to senior managers as appropriate.
  • To ensure that all staff delivering this programme are trained and supported in terms of Safeguarding policy and practice and that ATCIO’s Safeguarding Policies and practice are followed.
  • Assist the Strategic Locality Lead to prepare bids and grant proposals.

Collaboration between sectors

  • To provide a link to and work collaboratively with the wider work of at Action Together to deliver Social Prescribing, being a key part of the Action Together locality management team, taking on additional pieces of work as required.
  • Championing the role of the VCFSE, and local communities in all aspects of local delivery, and within a social prescribing context.
  • Working with the public and voluntary sector to ensure that the insight we gather affects local planning and delivery of services and removes barriers for people to engage in support.
  • Develop strong local (and where appropriate regional/national relationships) with staff, managers, commissioners and strategic leads across the health, care and public service economy working collaboratively to deliver the best outcomes for local people.
  • Be an active partner, representing Action Together in planning and delivery structures as required and create a positive, solution focussed partnership working environment in line with Action Together’s values.
  • Deputise for the Strategic Locality Lead as required.

About You

The ideal candidate for this role will be someone who:

  • Is driven by social justice and has an in-depth understanding of the VCFSE and of delivering person centred services.
  • Has great interpersonal skills and can foster and maintain effective relationships internally and externally.
  • Is an experienced line manager who can coach people to reach their full potential. 
  • Is experienced at operational and project management, assuring we achieve a high quality of service and performance to agreed timeframes.
  • Is a skilled influencer who can operate within partnerships and bring solutions.
  • Is an excellent communicator in person; in writing and effective in producing reports ad presentations to a wide variety of audiences.
  • Is outcomes driven and able to flex and prioritise to meet agreed commitments.
  • Experience in the way that health and care services work, and the limitations of the medical model.
  • Someone who genuinely understands the role that the VCFSE has in supporting people with all aspect of their lives is committed to person centred service delivery, inclusion and social justice.
  • Someone who is practical and can also look to the strategic context of the programme.
