We are looking to recruit a highly motivated individual to join our busy Hospital Aftercare team providing support to Salford Citizens 55+ who have been recently discharged from Salford Royal Hospital, back to their own homes. By providing short term practical support, needs lead assessments and support planning, facilitating access to other health and social care providers in order to support their recovery, regain independence and improve overall well-being whilst reducing the potential risk of hospital re-admission.
The service operates Monday-Friday although there may be the need to provide the occasional early evening/weekend cover.
We offer extensive health and pension scheme to the successful candidate.
All applicants must have a full driving licence and use of a car for work.
