Investing in Salford’s Strength: Live Well Supports Wellbeing Matters Social Prescribing

Live Well will provide investment in Wellbeing Matters Social Prescribing in Salford. Live Well is investing in a strengthened, community-led approach to adult social care in Salford through the introduction of three Wellbeing Matters Live Well Community Connectors. These roles will sit directly within the adult social care framework and work together with the contact team, social workers, and occupational therapists. Each Community Connector will also bring a specific skill set shaped around the unique needs of different Salford neighbourhoods. This tailoring ensures that the support provided is relevant and responsive to the lived experience of residents.

The Community Connectors will be fully embedded within the established Wellbeing Matters programme delivered by Salford CVS and VCSE partners. This programme has already demonstrated its value in building confidence, strengthening social connections, and empowering residents to take control of their own wellbeing. The new Community Connectors will enhance this work further by acting as a crucial link between adult social care and wider community support. They will work closely with primary care teams, mental health services, skills and employment support, and hospital teams, ensuring individuals receive coordinated, non-clinical and preventative support alongside any statutory care they may require.

To ensure that capacity within the VCSE sector remains strong and sustainable, a key principle of this initiative is ensuring that the delivery of the Live Well plan is rooted in strong partnership with Salford’s VCSE sector. Community organisations are often the first point of contact for residents seeking support, and they play a vital role in reaching those who may not engage with statutory services. By aligning adult social care with community strengths, this initiative aims to create a more preventative, connected, and compassionate approach to supporting residents.

Overall, this investment represents a meaningful step forward for Salford. By integrating Community Connectors into adult social care and strengthening the VCSE sector through dedicated development support, we aim to improve wellbeing, reduce pressure on statutory services, and build stronger, healthier communities across the city.
