We are looking for a motivated and compassionate individual with experience supporting older people to join our Home from Hospital team. This role supports individuals transitioning safely from hospital back into their own homes.
Key Responsibilities:
- Support safe and timely hospital discharges
- Visit individuals at home following discharge, ensuring comfort and wellbeing
- Provide practical and emotional support (e.g. shopping, prescriptions, utilities)
- Carry out small practical tasks (e.g. fitting key safes/handrails)
- Promote independence and wellbeing
- Signpost and refer to other services as needed
- Maintain accurate records and follow lone-working procedures
- Work as part of a team to deliver a responsive, person-centred service
- Occasional on-call cover (enhancement paid)
- In some cases, transport individuals’ home (mileage/company vehicle provided)
About You:
- Experience working with older people or in a support role
- Strong communication and organisational skills
- Able to work independently and as part of a team
- Practical, flexible, and reliable
- Full driving licence (required)
